Job Summary:
We are seeking skilled and compassionate Registered Nurses (RNs) to join our team and provide routine visits to hospice patients in their homes or residential care facilities.  This role ensures patients receive symptom management, comfort care, and emotional support while collaborating with the interdisciplinary team. 

Key Responsibilities:

  1. Perform routine nursing visits to hospice patients to assess their condition and provide care as specified in the POC.

  2. Administer medications, perform wound care, and monitor vital signs to ensure patient comfort and safety.

  3. Document all patient care and observations accurately and timely in the electronic medical record (EMR) system.

  4. Identify and report changes in the patient’s condition to the RN Case Manager, Physician, or Director of Nursing (DON).

  5. Educate patients and their caregivers on symptom management, medication administration, and hospice philosophy.

  6. Collaborate with the interdisciplinary team to adjust care plans as needed to meet patient and family goals.

  7. Adhere to all hospice policies, procedures, and federal/state regulations.

  8. Provide compassionate emotional support and education to patients and their families during visits.

Qualifications:

  • Current and unrestricted RN license in the state of Texas.

  • Minimum of one year of clinical nursing experience, preferably in hospice, home health, or palliative care.

  • Excellent communication and interpersonal skills to build relationships with patients, families, and interdisciplinary team members.

  • Valid driver’s license and reliable transportation.

  • Proficiency in electronic medical record (EMR) systems is preferred.

  • CPR certification.

Schedule and Compensation:

  • Flexible schedule based on patient needs.

  • $95 per completed routine visit.

  • Part-time and per-diem opportunities available.
