The LVN – Hospice Routine Visits is responsible for delivering nursing care to hospice patients in their homes or residential care facilities. This position ensures the implementation of the patient’s individualized plan of care, collaborates with the interdisciplinary team, and provides emotional support to patients and their families.

Key Responsibilities:

  1. Conduct routine nursing visits to hospice patients, following the established Plan of Care (POC).

  2. Assess the patient’s condition during each visit, documenting findings and any changes in the patient’s status.

  3. Provide basic nursing care, such as administering medications, monitoring vital signs, and performing wound care as needed.

  4. Report changes in the patient’s condition to the RN Case Manager, Physician, or Director of Nursing (DON).

  5. Communicate effectively with the interdisciplinary team, patients, and families to ensure continuity of care.

  6. Educate patients and caregivers about medication, symptom management, and hospice services.

  7. Maintain accurate and timely documentation in accordance with hospice policies and regulatory standards.

  8. Adhere to Better Days Hospice policies and procedures, as well as state and federal regulations.

  9. Provide compassionate emotional support to patients and their families during routine visits.

Qualifications:

  • Current and active LVN license in the state of Texas.

  • Minimum of one year of clinical nursing experience, preferably in hospice, home health, or palliative care.

  • Valid driver’s license and reliable transportation.

  • CPR certification.

  • Excellent communication and interpersonal skills.

  • Strong organizational and time-management skills.

  • Proficiency in using electronic medical record (EMR) systems is preferred.

  • Flexible schedule with visits assigned based on patient needs.

  • $50 per completed routine visit.

  • Part-time and per-diem opportunities available.
